The Court’s local forms are grouped into the following categories: Required Forms, Clerk’s Office Forms, and Recommended Forms.
Updated Recommended Forms have now been posted to the Court’s website (www.nmb.uscourts.gov) under the red tab labeled “Forms & Local Rules.” In addition, several of the Recommended Forms may be also be obtained from the General Chambers’ Procedures page, under the red tab labeled “Judges & Opinions.” All local forms are available in Microsoft Word (doc) and portable document (pdf) formats. The forms are numbered to correspond to the related Bankruptcy Rule or Bankruptcy Code Section.
Use of the recommended forms is encouraged, but not required.
