In New Mexico, it is the responsibility of the self-represented debtor to check the "Notice of Chapter ___ Bankruptcy Case, Meeting of Creditors, and Deadlines" within 60 days of submission of the statement of social security number(s) to be sure the correct number(s) were displayed on the Notice. Prepare and file an Your Statement About Your Social Security Number(s) (AMENDED). (note: this form is a pdf fillable form; this means you can complete it online and print it for signature)
- The debtor should prepare and sign an Amended Statement of Social Security Number and send it to the Clerk's Office, U. S. Bankruptcy Court, District of New Mexico, 333 Lomas Blvd. NW, Albuquerque, NM 87102.
- Send a Notice of Corrected Social Security Number to all creditors and parties of record in the case and file a Certificate of Mailing of Notice of Corrected Social Security Number with the Clerk's Office.
